The Sexual Health Tariff Grouper is offered on an annual subscription basis. Subscriptions are assigned to named individuals (not departments or organisations). Please compare subscriptions.

| 1. To request a subscription, please login to our website so we can validate your email address: |

| 3. When you are logged in please click this red subscribe button: |

| 4. Or, if you are renewing a subscription, click My Subscriptions from the menu. Select your expired subscription and click the green renew button. |
| 5. If this is a new subscription, select the subscription you require and complete the online form (your organisation name, billing address, telephone number and purchase order if you have one). |
| 6. We will receive your request and activate your account. Your subscription will start from the date we activate your account and expire after 12 months unless we state otherwise. |
| 7. If this is a new subscription, we will send you your login credentials for the Grouper. |
| 8. You will also receive an invoice (we can update this with a purchase order if you send one later, (our company details are available). |
| 9. Use of the service is subject to our Subscriber Agreement. |
