Impact assessment of funding changes relating to Early Supported Discharge (ESD) of stroke patients from hospital; and identification of funding options to support stroke pathway reconfiguration across hospital and community settings.

Working with clinicians from two hospitals and multiple community teams involved in the care of stroke patients in South London, we ran an impact assessment on possible ESD improvements brought about by funding changes. We built a series of interlinking economic models to enable a detailed break-even analysis of ESD across hospital-community pathways and modelled the impact on community resources of incremental changes in ESD processes - for example, potential reduction in hospital rehab costs vs. the potential uplift in community costs resulting from the new model of community care.
